Hospice & Palliative Care · Burlington, VT · Est. 2011

The final chapter deserves the same care as every chapter before it.

Capstone Care Health provides hospice and palliative care built around clear communication, comfort, and honoring what each patient actually wants — for the patient, and for the family walking alongside them.

A capstone doesn't just finish an arch. It's what holds the rest of it up.

Hospice, Simply Explained

Hospice isn't giving up. It's choosing comfort over a cure that isn't coming.

Hospice care is available once curative treatment is no longer the goal, typically when a physician certifies a prognosis of six months or less. It doesn't mean less care — it means a full interdisciplinary team, on call around the clock, focused entirely on comfort, dignity, and quality of the time that's left.

Our Care

Our Story

Dr. Helena Brennan started here because of her own father.

She watched him move through his final months in and out of hospitals, given treatment that no longer had any real chance of working, with no one ever quite sitting the family down to ask what he actually wanted. Capstone Care Health exists so that conversation happens early, clearly, and with real support behind it.
